Presenting Symptoms

It may be helpful for your psychologist to have an indication of your presenting symptoms before your initial appointment so they can more accurately prepare for the session. If you have yet to be placed with a psychologist, this section will help reception staff to better match you with a practitioner who specialises in that area.

If you're unsure or you're not comfortable answering this section, please leave it blank.

Feel free to select all options which you feel relate to your present condition.

Cancellation Policy

Sometimes there are good reasons for cancelling appointments however unlike brief medical appointments it is impracticable to fill a cancelled psychology appointment that runs for 50 minutes.

A fee of 50% of the appointment cost will be charged when you fail to cancel or change your appointment before 3pm on the previous business day, or you fail to attend the appointment. This means for a Monday appointment, notice must be given on Friday (as reception is staffed Monday-Friday 9am-5pm). For assessment services we require two business days' notice. Cancellation fees are an out of pocket expense: they are not covered by insurance and are unable to be rebated through Medicare.

If you are running late, please call and let us know to avoid any fees. If you have COVID-19 symptoms, please reschedule or alternatively you may opt to have your session via Telehealth (video or phone).

Fees are payable on the phone at the time of cancellation or via a secure payment link. Future appointments will be forfieted if the cancellation is not paid.

Please note a late cancelled appointment is a loss to three people:

1. The client who is delaying their therapy progress;
2. Another client who has been on the waiting list to see their psychologist urgently;
3. The psychologist who spent the morning preparing for the session.
